The earliest one I can remember is Superman #11, I believe where he's battling Mxyzptlk for the first time in Byrne's run.

Lothor -- Frankly, I have no idea whether the Pre-COIE Mxyzptlk ever showed signs of knowing who Batman really was.

Oh, and Byrne DID change Mxyzptlk a bit, by removing the fixed "name game" conditions for his banishment.
