One I use a lot for really bad cases is a mixture of Benedryl liquid, viscous xylocaine, and maalox, all mixed together.
As a side note, due to my resistance to pain killers, he now automatically gives me two ampoules (a double dose) of xylocaine or septocaine or whatever it is he uses in the needle.
