Koothrappali is a place in the district Kottayam of state Kerala of india. It is quite common for people from Kerala( I'm also from there so I know) to add the name of your home town to your name. Especially if you are famous.
But it's kind of not proper because Raj is portrayed as a north Indian guy.. (Hindi speaking) whereas we from south don't speak Hindi.
Ooh and in our language (malayam ., of kerala.) Koothra means a culturless/ shameless/ stupid person / thing.. You know what I mean. . We call people Koothra like you say ' you jerk' or ' you dumbass '.
But I think it's the place name they mean.
