I also was a bit confused about his scene with Mark Ruffalo in the abandoned building by the cemetery where Ruffalo suggested they (Shutter Island/Dr. Crawley or whoever was "doing experiments") were looking into him, because seeing as he was Leo's psychiatrist, I don't understand why he would put that thought into his head at all - it would only further put him out of sanity.
